The Salisbury Fire Department is always looking for ways to improve our service delivery to our community. We have created our “Letter of Life” form in an effort to maximize our time on emergency medical services calls.
We encourage all residents to print out and complete one form for each member of your household, especially the elderly and persons with chronic conditions. It can be found here or on the Fire Department section of the City of Salisbury website under “Documents.”
This form will allow our Emergency Medical Services providers quick access to any pertinent medical history or conditions that may need to be addressed.
We suggest the form(s) be located either on your refrigerator or the back of the entry door to the residence. We also suggest you attach a copy of the Maryland MOLST or DNR, as well as a copy of your most recent ECG from your Cardiologist.
