    MPIA Request: Environmental Assessments

Use this form to request information for environmental assessment. (For companies seeking Phase 1 documentation, underground tank information, etc.)

    • Please indicate the nature of the information you're seeking. It is not uncommon for structures and/or properties to have no environmental issues or fire history due to their not having experienced any such issue in their lifetime. In such case, we will respond and indicate that the structure/property in question has no documented issues.
    • Use this field to indicate the street address of the property for which you seek documentation.
    • Use this field to further describe the documents/information you are requesting.
How would you like to view and/or receive the requested documents?
    • This field is for validation purposes and should be left unchanged.

If the requested documents are available, the City of Salisbury will make every effort to produce them as soon as possible within the thirty (30) days allowed by the Maryland Public Information Act.  If we anticipate that your request may be denied for any reason, you will be notified within ten (10) days of receipt of your request.

If your request requires more than two (2) hours for City employees to research and compile, you may be charged for the employee’s time after the first two hours. You will be notified of the estimated cost and, if the scope of work is extensive, a good faith deposit may be required, as allowed by the MPIA.
